The purpose of this USCIS document is to prove that applicants for adjustment of status, or Green Card, are not inadmissible due to health-related reasons. These medical conditions include infectious diseases, like Tuberculosis (TB), or lack of required vaccinations like Measles and Hepatitis B. However, the COVID-19 vaccine is no longer required.
Yes. I-693 is now required for most individuals adjusting their status to lawful permanent resident within the U.S. However, refugees and asylum-seekers may have different requirements, but usually do need to submit a proof of vaccination record.
The I-693 form must be completed by a USCIS-authorized civil surgeon to confirm you have the required vaccinations and to check for signs of communicable diseases. The doctor will provide you with a sealed envelope containing the results of their examination. You must not open it or your application would very likely be rejected.
Lawful permanent resident applicants, including family-based, employment-based, and Diversity Visa winners, must provide the USCIS with the sealed envelope containing form I-693. You would then submit the form along with Form I-485, Application to Register Permanent Residence or Adjust Status. Form I-693 is valid only for the specific pending application for which it was completed, replacing the previous indefinite validity policy. Although properly completed forms signed on or after Nov. 1, 2023, are valid indefinitely.
